CUSTOMIZED COMMUNITY SUPPORTS

Supported Living

Supported Living services are provided in groups of four or fewer in the individual’s home and community in accordance with the ISP.  Supported Living services must be available up to 24 hours per day 365 days per year, as determined by the IDT, but shall not include time when the individual is in an employment, school, or customized community support service. In addition, necessary levels of staffing must be provided in accordance with reducing the risk of self and others as supported in the ISP. Staff in various personal and social skill areas furnishes training as identified by a comprehensive assessment process and as specified in the Individual Service Plan.  Structured activities will be accompanied by the provision of support services necessary to improve an individual’s quality of life.  Implementation of meaningful day and outcomes as outlined in the ISP will be supported in the provision of Living Supports.

CUSTOMIZED COMMUNITY SUPPORTS

Individual & Group

Measurable individual progress as specified in the ISP, including the individual’s personal definition of a meaningful day, their preferences and progress towards outcomes in the ISP.  The designation of this service is to promote independence providing needed support and opportunity for each individual to become an integral participant in his/her community in the manner desired by the individual/group.  During the ISP, the IDT members are required to offer employment as a priority over other day service options, while concurrently supporting the development of valued social relationships. This all while measuring progress towards outcomes identified in the Individual service plan.  This includes the development of skills, and behavior that strengthens interpersonal relationships as well as an individual/group’s connection with his or her community.  The objective of is to create meaningful, sustained relationships with “non-disabled” community members who share common interests, preferences, and goals. The individual/group is supported to create social networks in a variety of settings. Following selection of the aforementioned service Links conducts a profile. This profile is feedback collected from the individual served, the ISP and IDT members. It provided an in-depth snapshot of likes, dislikes, interests, hobbies, goals, communication and learning style. With this information Links is best able to determine compatibility between those individuals receiving CCS Group, all while providing valuable information in supporting the individuals meaningful day. Providing the most individualized integrated service. This service is supported by the Links Master Staffing Plan.

CUSTOMIZED IN-HOME SUPPORTS

A Personal Approach

Customized In-Home Supports services is an individual intervention and support service, which promotes a more independent environment and life style.  Customized In-Home Supports are only provided in the recipient’s home or home of an unpaid family member or natural support.  Staff support is available as needed and is furnished on a planned periodic schedule of less than 24 hours per day as required in the Individual Service Plan.  Unscheduled staff support may be available through an on-call system and as described in the ISP approved by the Interdisciplinary Team. CHS is not a residential service but a support for those requiring less supports than those under Living Support Services. The person receiving this service will generally not be a health and safety risk to themselves or others.
